Regarding Insurance: I do not bill insurance for your treatment. Payment is at the time of service and I accept checks, cash, debit and credit cards such as MasterCard, VISA, and Discover Card. If you have insurance I can provide you with a superbill receipt (which has the proper CPT & ICD-9 codes) that you can submit to your insurance carrier for possible reimbursement for your treatment with me.
